Before Your Visit
If you are a new patient, please fill out the registration forms listed below in advance of your appointment to assist the staff in making sure that we have all the information necessary to provide you with quality care and treatment.
Please provide the following information when you arrive for your visit:
Your photo identification and child’s health insurance card(s)
Complete immunization records
Medical records from your previous pediatrician
Insurance Processing
We accept most health insurance plans and will file claims on your behalf, saving you the time and hassle. You will need to confirm your insurance acceptance when you make your appointment. If you have a change in your insurance, please notify our office at least 24 hours before your appointment.
To completely understand what is covered by your health benefit plan, it is best to contact your employer or insurance company. For services not covered or not submitted to insurance, we accept cash and all major credit cards at the time of service.
